Note:

Once each of the company drivers have completed all four home run for safety programs:
defensive driving, fatigue management, MCDD skid pad training, professional driver coaching,
AND the company has participated in management training; all of the requirements for the
Home Run for Safety Award have been met.

The award is presented at the annual Michigan Safety banquet scheduled in February
each year. Complete details will be provided once we receive the completed form.
